The Care Team

Medical Director

Our Hospice and Palliative Care Certified Medical Director does not take the place of the attending physician but is available for pain and symptom management, or in the absence of the attending physician.

RN Case Manager

Our nurses provide individualized care focusing on symptom management and comfort care.

Certified Nurse’s Assistant

Our CNAs provide personal care and assist with the activities of daily living ensuring dignity and quality of life.

Chaplain

Chaplain Services are available to provide emotional and spiritual support.

Social Worker

Our Social Workers assist families with resources and referrals as well as support in coping with the life-limiting illness.

Volunteers

All of our volunteers are fully trained and available to provide friendly visiting services.

Infinity Hospice Foundation

The Infinity Hospice Foundation is a non-profit organization dedicated to providing education to Physicians, caregivers and family members of the terminally ill regarding hospice and palliative care.
