Infinity Hospice Care enhances the quality of a patient’s life by emphasizing the dignity of the individual and the strength of the family unit. Under the supervision of the medical director and attending physician, nurses, social workers, chaplains, counselors, and home health aides perform as a team to ensure the highest quality of life for the patient. We provide a combination of medical, spiritual and emotional care. Every effort is made to ensure that the patient remain at home during their illness. When home is not enough, inpatient care is arranged with one of our contracted facilities.
This level of care includes all hospice services that are usual and customary.
The basic premise of hospice is to provide support and services to enable the patient to remain at home. There are times when the patient's condition cannot be managed in the home setting. This increased level of care is called Inpatient Care and is provided at approved facilities that meet the requirements for Inpatient level of care.
At times caregivers may need a break or a short period of rest. This is called Respite. The hospice nurse and/or social worker make arrangements for the patient to be placed in an approved facility for Respite Care.
This level of care provides services in the home during time of crisis or when acute needs are present that require increased assistance and skilled nursing assessment and management. This level of care is intended for around-the-clock 24/hr care during periods of crisis. The Interdisciplinary Team and family determine the need.
The Infinity Hospice Foundation is a non-profit organization dedicated to providing education to Physicians, caregivers and family members of the terminally ill regarding hospice and palliative care.
